COVER to COVER

Cover to cover logoCOVER to COVER "Connecting Older Veterans Especially Rural to Community or Veteran Eligible Resources" (C2C) was launched as a demonstration project in 2013 by the Utah Aging and Disability Resource Center (ADRC) as a collaborative effort with the Veterans Health Administration – Office of Rural Health. The program has been awarded on-going funding in partnership with the Utah Department of Health Primary Care and Rural Health for specific rural agencies. The model brings community agencies and VA staff together through training to equip staff to better serve their Veteran clients and to build partnerships between agencies.

  • ​​COVER to COVER bridges knowledge gaps about VA eligibility and enrollment by training and educating community based agency staff in VA benefits and processes.
  • ​Veterans can get help with navigational support, guidance, advocacy, and benefits counseling. Results could lead to access of the broad and increasing health, program, and service benefits that the VA provides.
